Abstract

Biography

Alison is the Portfolio Manager for Chronic Disease Services at Marathon Health, NSW. As an experienced credentialled diabetes educator, Alison is passionate about improving health outcomes for people living with type 2 diabetes in the bush. Alison and her team, which includes five diabetes educators work collaboratively with GP practices and Aboriginal Medical Services across Western NSW to deliver client centred, culturally safe and coordinated multidisciplinary self-management support for people living with type 2 diabetes. She has been involved in the ACI Diabetes in the community for Aboriginal People project (DiCAPP), is a clinical Advisory Council member for the Western NSW PHN and is also a member of the Statewide Initiative for Diabetes Management Amalgamated Implementation Working Group and the Western NSW Care-Partnership Diabetes Workforce Working Group.

Biography

Jaclyn is a Senior Accredited Practising Dietitian and Credentialled Diabetes Educator working with Marathon Health, based out in the Central West of NSW where she was born and raised. She is privileged to work with people in her own backyard, offering practical, down-to-earth support for managing nutrition and diabetes. Making health care more accessible in the bush is a topic close to her heart and she genuinely loves being part of that journey.

• Registered nurse for 42yrs training at Dubbo Base Hospital • Paediatric Certificate - Adelaide Children’s Hospital in 1989. • Midwifery - Lyell McEwan Hospital Adelaide in 1992. • District Nursing in Orange for 7yrs. • Low care aged facility, for 7yrs. • Practice Nurse for 4 yrs. • Aboriginal Medical Service. • Care coordinator for 5 yrs in Bathurst and Orange. • Certificate of Diabetes education – Curtin University 2020 • 2020 Credentialling achieved • 2019 to now providing outreach DE services in rural NSW • Passion – T2DM, First Nations health, patient advocacy and commitment to improving health and wellbeing of regional Australians.

Biography

Erin is an Accredited Practising Dietitian and Diabetes Educator working with Marathon Health based in Central West NSW. As a local to the area, she is passionate about providing healthcare to people in regional areas with limited access to services located in larger towns and cities. She provides dietetics and diabetes education services to 8 regional towns, and works closely with local GP practices to provide nutrition and diabetes care.

Biography

Deanne Minniecon is an Aboriginal (Goreng Goreng) and Torres Strait Islander (Erub) woman with 30 years’ experience working in Aboriginal and Torres Strait Islander health and education. Deanne holds a Master of Health Science (Health Promotion) and is currently employed as Group Executive, Aboriginal and Torres Strait Islander Engagement with Diabetes Australia. Deanne holds an Adjunct Associate Professor in the Faculty of Health, School of Exercise & Nutrition Sciences at QUT. Deanne is also on the Board of West Moreton Hospital and Health Service, the Darling Downs & West Moreton PHN and the Indigenous Wellbeing Service in Bundaberg.

Biography

Amanda is a credentialed pharmacist with over 20 years of experience in the community pharmacy space. Currently working for Marathon Health as a Credentialed Diabetes Educator and providing pharmacist services into General Practices, Amanda provides outreach clinics to 6 different communities throughout Central West NSW. Amanda is passionate about providing culturally safe and respectful care in the management of Type 2 diabetes to clients. She enjoys working in collaborative environments to improve health outcomes and working towards reducing the health gap for our First Nations people living with Type 2 diabetes.

Biography

Karen Ingram is a Wiradjuri women from Leeton in NSW. Karen has worked for Marathon Health for almost 9 years. She started as a Care Coordinator and Project officer for the ITC program. Marathon Health identified the importance of having a First Nations person to support their journey to Reconciliation, Cultural Safety and workforce development for First Nations Community and Staff. Karen applied for the role as Cultural Capability Lead and has been in this role over the past 18 months. Her role is to provide a First Nations voice giving expert advice and leadership. Karen is very passionate about the close the gap targets and working with strong allies across Marathon Health to deliver Culturally safe and accessible services to my people. It is a priority to embed self-determination into design and implementation of programs with deep-listening, shared decision making enabling First Nations peoples to lead their own health journey.
