For people living with diabetes, access to a person-centred team that supports them to optimise their glycaemic management, prevent complications and improve their quality of life is the dream. As 80% of diabetes care occurs in primary care, more is needed to enable people living with diabetes to access the primary care they need, where, how and when they need it.

Biography
Lauren is a dedicated professional with a passion for health promotion, improving lives, community engagement & advocacy for all people living with diabetes. As the Health Promotion & Communications Manager at PDC Health Hub, she develops impactful communications, programs and services for the diabetes community. Having been diagnosed with Type 1 diabetes at the age of 28, and growing up with a mother with T1D, Lauren brings her lived experience and deep understanding of the condition to her work each day. She is an active member of the T1D Collective, a peer support and advocacy group. She also leads numerous advocacy projects and is an active member of steering groups working tirelessly to bring awareness and positive change to the world of diabetes.

Biography
Dr Jake O'Brien is a Medical Doctor, immediate past Chair of the PDC Type 1 Diabetes Collective, and Care Coordinator at PDC Health Hub. Having lived with type 1 diabetes for 9 years, Jake brings the unique perspective of both personal lived experience and professional knowledge to his medical practice. Jake firmly believes that patients are the experts in their own medical conditions, and should be empowered and supported in their overall management.

Biography
Early in her clinical podiatry career, Rajna identified gaps in clinical practice and care outcomes leading her to embark on a research career to prevent foot problems in people with diabetes. Her research has evolved, recognizing the need to consider individuals holistic wellbeing and the crucial role of implementation, including the need to actively engage all stakeholders in the journey. She currently works as a Senior Research Fellow at Bolton Clarke Research Institute and her research aims to optimise health and wellbeing through co-design and community based, person-centred collaborative care, and is Chair of Primary Care Diabetes Society of Australia.

Biography
Nicole is an Accredited Exercise Physiologist and the Director of Exercise for Rehabilitation & Health, Essendon Physio Group and the Melbourne Vestibular Clinic. With a focus on innovative health care solutions at a population level, Nicole has pioneered programs in diabetes prevention and management as well vestibular assessment and rehabilitation. Her commitment to evidence-based practice and person-centred care has earned her numerous accolades, including the 2023 Australian Women's Business Champion award and ESSA practice of the Year Award. Nicole’s work aims to improve health outcomes through strategic partnerships, advocacy, and cutting-edge healthcare solutions. She is also a board member of the Primary Care Diabetes Society of Australia, promoting best practice in diabetes care.

Biography
Jeff Au is a co-director of PDC Health Hub in Perth, a credentialled diabetes educator, and a pharmacist with a particular focus on men's health and diabetes technology. He champions open discussions and is passionate about empowering individuals to take charge of their health through preventive care and access to innovative diabetes management tools.

Biography
Dr Ashley Ng is a diabetes advocate, a diabetes researcher and accredited practising dietitian. Her lived experience has led to her passion in consumer and community involvement in research and healthcare improvement. Ashley's areas of interest in research include supporting young adults with diabetes during life transitions, digital health solutions in diabetes care and community engagement, including peer support.
