Biography

Rachel McKeown is an Advanced Accredited Practising Dietitian and Credentialled Diabetes Educator. She currently works at Mudgee Health Service, NSW in the role of diabetes educator. Rachel has over 15 years experience in the diabetes education field and has been an APD for over 20 years. She has worked in public and private health sectors, as well as NGOs. Her current client base involves the range of diabetes management and support for all ages in her community, and for all types of diabetes, working closely with GPs, remotely with Endocrinologists, allied health, nursing staff and linking in with tertiary services as required. Her previous role at the Australian Diabetes Educators Association as Professional Services and Education Manager included the development and revision of numerous diabetes related policies and guidelines, including national competencies for credentialled diabetes educators and the credentialling pathway.

A/Prof Sham Acharya is the lead investigator for Diabetes Alliance Program Plus, a NSW Health award winning, integrated model of care for diabetes management, which has been transformative in improving clinical outcomes.

Biography

Gael Holters is a dedicated Certified Diabetes Educator (CDE) renowned for her passion and expertise in improving the lives of those with diabetes. Currently Gael is coordinating the DREAM Initiative-a rural and regional program between the Murrumbidgee Region and St Vincent's Hospital, Sydney.
